Update an Entry
If your organization is already in the database, you can update your information here. Contact the Database Department if you need your username or password.
Note: If you are looking to update information about a 590 Team, you must have DMH distribute that information to the 988 call centers on your behalf. Reach out to your DMH contact person directly.
Join PATH’s Database
- Check your agency’s eligibility. There is no cost to be included in PATH’s database, but inclusion must be justified based on PATH’s Inclusion/Exclusion policies. Guidelines differ slightly depending on whether you serve our 211 or 988 coverage areas.
- Submit a New Agency Form. Database staff will review the details submitted and reach out to you as soon as possible to confirm your agency’s inclusion, ask any relevant questions, or explain the reason for exclusion. Please see this workflow to learn more about PATH's new agency process.
