CICoC

Central Illinois Continuum of Care (CICoC) Planning and CoC Lead Organization

  • Operating and planning for the CoC as laid out in the U.S. Department of Housing and Urban Development's (HUD) CoC Interim Rule. Our CoC includes 11 counties: DeWitt, Ford, Iroquois, Kankakee, Livingston, Logan, Mason, McLean, Menard, Piatt, and Vermilion
  • PATH submits collaborative applications on behalf of the CICoC for the following grants: Continuum of Care (CoC), Emergency Solutions Grants (ESG), Homeless Prevention from Illinois Department of Human Services (IDHS)
  • As CoC Lead, PATH will monitor CoC and ESG-funded projects to ensure compliance with regulations, and we are effectively and efficiently assisting those experiencing homelessness
  • Identify gaps in services in the CoC and find solutions to those gaps in services as well as improve existing services
  • Plan and organize the annual Point-in-Time (PIT) count and corresponding Housing Inventory Count (HIC). Then submit that data to the U.S. Department of Housing and Urban Development (HUD).
  • Hold CICoC meetings as well as meetings for the McLean County CoC Hub

 


Coordinated Entry System

  • The main point of contact for the CICoC's Coordinated Entry System, which spans 11 counties in the central Illinois area
  • Assess people experiencing homelessness to get on our Prioritization List for housing
  • Maintain and update the Prioritization List for housing
  • Make referrals to housing projects that participate in our Coordinated Entry System

 


Homeless Management Information System (HMIS) Lead Organization

  • PATH maintains and operates the CICoC's HMIS database, which securely stores confidential client information for all CoC and ESG funded projects. Other projects in the CoC that HUD does not support are encouraged to participate in the HMIS.
  • Train all project users for HMIS on data entry and data quality across the CICoC
  • Provide data to the U.S. Department of Housing and Urban Development (HUD) from HMIS for the Longitudinal Systems Analysis (LSA) and System Performance Measures (SPM).
